Please complete this form and return to Church Administrator one month in advance of the event. This will provide the Music/Arts/Drama Ministry sufficient time to plan and coordinate your request. Be reminded, if this form is not returned in the appropriate amount of time there is NO GUARANTEE that you will have all functional items on the day of event.
